The case story below describes how acupuncture and Tuina helped a young man return to free movement, and what it can teach us about treating long-standing shoulder pain.

The Story: Two Years Without Lifting His Arm

Cases like this are the ones I love receiving — they are a great challenge for me as a practitioner, and exactly the kind I trained for. When I asked him how long he had been suffering, he answered, “Two years.” He had already seen some of the country’s leading orthopedists, and they had referred him for shoulder surgery. He had also tried acupuncture at one of the health-fund clinics, but it did not help and was even painful. Eventually, someone he knew recommended he come see me. “What do I have to lose,” he told me, “at most I end up at shoulder surgery anyway.”

The First Treatment

His treatment involved just a single needle using the “Machat Ruach” technique, along with about ten minutes of Tuina massage to the shoulder area. I applied pressure to several points, some local to the shoulder itself and some located entirely elsewhere on the body. At the end of the treatment I asked him to try lifting his arm. “Wow, I haven’t been able to make that movement in two years,” he said, surprised. It was a moment of real satisfaction, for both of us.

What Lies Behind Long-Standing Shoulder Pain

Shoulder pain that lasts for months or even years often stems from a combination of tension in the tendons and muscles surrounding the joint, alongside a restriction in mobility that worsens over time (sometimes called “frozen shoulder”). In Chinese medicine, the shoulder area is crossed by several important meridians — including the large intestine, small intestine and gallbladder meridians — and pain in this area is usually understood as stagnation of Qi and blood in one or more of them. The longer this stagnation continues without appropriate treatment, the more it can deepen and make full recovery of range of motion more difficult.

The Acupuncture and Tuina Approach to Shoulder Pain

The treatment approach combines local acupuncture points, placed directly in the painful shoulder area, together with distal points that can directly affect it through the meridians connecting them. Tuina massage complements the acupuncture by releasing tension in the muscles and tendons around the joint, and helps improve blood and energy flow in the area. This combination is designed to address both the immediate pain and the deeper cause holding it in place.

Why a Single Needle Was Enough

One of the questions patients ask me most often is why, sometimes, a single needle is enough to bring about such a significant change. The answer has to do with precisely selecting the point: when the meridian through which the blockage runs is correctly identified, even a small, precise stimulus can release it and let Qi and blood flow again. It does not always work this quickly, and for other patients several sessions are needed before a meaningful change is felt, but this case nicely illustrates the underlying principle of the method.

When Pain Persists Over Time

Chronic pain that accompanies a person over a long period affects not only the body but also mood and self-confidence, especially when it limits simple actions like raising an arm. It is important to remember that Chinese medicine is a complementary treatment and not a substitute for medical diagnosis and treatment, so it is worth combining the two and consulting a specialist physician when the pain is severe or prolonged. At the same time, as this story illustrates, a meaningful change in mobility and pain can sometimes begin as early as the first treatment.
